Lidocaine HCl
Indications/Uses
Surface anesth & lubricant for male & female urethra during cystoscopy, catheterization, exploration by sound, endourethral operations, nasal & pharyngeal cavities in endoscopic procedures (eg, gastroscopy & broncoscopy) during proctoscopy & rectoscopy & tracheal intubation. Symptomatic treatment of pain in cystitis & urethritis. Pain relief after circumcision in childn.
Dosage/Direction for Use
Urethral anaesth Male 20 mL. 30-40 mL in 3-4 portions when anaesth is especially important (eg, sounding or cytoscopy). Female 5-10 mL. Endoscopy 10-20 mL. Total max dose: Lidocaine 400 mg when combined w/ other lidocaine products. Proctoscopy & rectoscopy Up to 20 mL for anal & rectal procedures. Total max dose: Lidocaine 400 mg. Lubrication for endotracheal intubation 2 mL to surface of tube.
Contraindications
Hypersensitivity to amide anesth, methyl &/or propyl parahydroxybensoate (methyl-/propylparaben) or to their metabolite, PABA, ester local anaesth.
Special Precautions
Excessive doses of lidocaine products or short intervals between doses can result in high plasma levels & serious adverse effects. Presence of sepsis or severely traumatized mucosa in area of application. Oropharyngeal use has increased risk of aspiration. When used for endotracheal tube lubrication, avoid introducing into lumen of the tube. Closely supervise patients being treated w/ class III antiarrhythmic drugs eg, amiodarone & monitor ECG due to additive effects. Patients w/ partial or complete heart block, elderly & patients in poor general health. Porphyric patients. Advanced liver disease or severe renal function. Pregnancy & lactation.
Adverse Reactions
Intoxication, cutaneous & hypersensitivity symptoms.
Drug Interactions
Patients receiving agents structurally related to local anaesth, drugs reducing clearance of lidocaine eg, cimetidine & β-blockers.
